I read it can be offline only. But I don’t know which forms to download fill by both account holders and what docs need of both?
And is there a concept of nominee too in demat account?

Take an extra copy of pages 3&4 of the above doc.

Since the account you want is a joint account, you will notice that application has signature boxes marked in S series(S1,S2…) those have to be signed by the second holder, all the other details will be of first holder only. The extra two pages you have printed will have the second holders KYC details.
Documents, along with your proofs, the second holders self attested copy of PAN card and address proof will be required as well.
