how to place order with price higher than spot price?
U can use sl-l (stop-loss limit) or sl-m (stop-loss market) with price higher than current as d parameters… Eg icici cmp is 298 and u want to buy only when it breaks day high say 299, u may set sl-l and sl-m at 299.1. Hence ur buy wud b triggered only when price goes abt ur set parameters
Sl-l may not guarantee trade if price jumps suddenly from 298.95 to 299.5 sl-m will execute at 299.5… Hope its clr and wud b useful for u 
Correct. Use SL or SLM. However use SLM instead SL Bcos in SL, your order may not execute and will get skip if exact given price will not come. So it’s better to use SLM
