Dematerilising physical shares

i have recently found hard copy of Confirmation of Allocation Note(CAN) dated 26/3/2005 mentioning 34 equity shares of Punjab National Bank allocated to me.
Can anybody kindly suggest me how to transfer these shares to my current demat account.
